The coolest alarm watch I ever saw was worn by James Coburn as Derek Flint in "Our Man Flint" and "In Like Flint". It had a crown shaped like a " T ". The T would pop out and rock back and forth such that the arms (top of T) would press against the forearm waking him up.

There are some G-Shocks that vibrate. I think they are older models at least here in the States because I can't seem to find one in the current lineup.

The GW-400J G-Shock is currently available. I'm in the UK so I don't know if it's available in the US, but I can recommend some Japanese shippers whom the G-Shock forum all trust.

Reminds me of the watch that Queen Elizabeth I of England wore. It was mounted in a ring (presumably a rather large ring) and when the alarm was triggered, a tiny needle tapped against her finger.

My Poljot alarm watches vibrate somewhat, and can wake me without disturbing Mrs Chascomm:

Her Poljot however is a 1995 model with the 2-piece back and tunable anvil:

That one has quite a fierce buzz.

Those 'Flint' movies were a lot of fun.
