what should I be aware of when considering a customized rolex?
My understanding is you can't get a customized Rolex serviced by an authorized Rolex dealer. Not an insurmountable problem as there are plenty of good independent watchmakers who can service a Rolex, but that may be an issue. And if you think you might want to sell it one day, it will limit your resale market as well.

Haven't had experience with titanblack specifically, but in my experience these coatings are not very durable. Shows scratches/marks that can't be polished out. As dbt pointed out, resale market is not as big for customized items. Seeing as how this is your first luxury watch, it's hard to predict how your tastes may change, you might want to flip it eventually.

This customization voids the warranty anyway so I see no compelling reason to buy new. Therefore you can do this a lot more cheaply by picking up a pre-owned Milgauss and sending it off for the DLC yourself. These DLC watches are always shockingly overpriced to me.
