I’d say that it will first come out on the Fenix 8x and then be ported to the 7 series.

F8 is still about year or more away. They may pull it off on some premium or Forerunner 9XX models first.
Considering low levels of integration in Garmin watches they can pull it off very easy.
By low integration i mean in most smartwatches most of functions integrated into single chip, while Garmin uses array of chips: CPU, GPS, BT etc.
I have ECG on my Samsung and it been around since 2019. By this time almost anyone has it: Apple, Samsung, Fitbit.
Missing out are WearOS (apart from Samsung) and smaller manufacturers.
It's not that useful, opposite to some other metrics: blood pressure, glucose or accurate body temperature.

On other hand this may be already integrated into F7 and such and it only matter of software update.
It kind of about time Garmin announces something. So may be it will be it.

It officially out.
First model to get it is Garmin Venu 2 Plus.
It will get via new firmware and updated app.
Rest will follow.
So chances are Fenix 7 and all new high end Forerunners have it.

According to DC Rainmaker, the only current model that supports ECG is the Venu 2 Plus - all other current models lack the necessary hardware, so they will not get the ECG App software.

The missing hardware is a metal ring around the optical heart rate sensor on the back of the watch. The Venu 2 Plus is the only current model with this metal ring. Here's a pic:

This ring cannot be retrofit on current models that do not already have it, so we will have to wait for new/updated models of current offerings if we want ECG capability.

Garmin Fenix 7 has all metal back, but no separate ring. My Galaxy Watch Active 2 had some glass covered thing and no contacts visible (like ring) ECG works.
It should have connection to button to ensure close loop.
Considering Fenix 7 is later model and more advanced/expensive i would assume it should of had ECG planned and built in.
Time will show. May be Garmin will announce Fenix 7 plus or whatever to get more confusion and money.
Who knows.

Venu 2 Plus is a good model.

Here's Garmin's official response to questions about ECG:

ECG App

Currently it is US only and Venu 2 Plus only - other models are hardware limited and ECG App is not supported.

Instructions are 1:1 these on Samsung and same detection/limitation/how to use things.

Usually the “X” size gets the latest upgrade. Will be interesting to see if all the 8’s get it or just the “X” series and the other ones get the Flashlight.
